Welcome to Dallas Podiatry Works. Our office is dedicated to the health and comfort of your feet and ankles. You don’t have to suffer with pain in your lowest limbs—foot problems can be treated. If you’ve already made an appointment with us, whether as a returning patient or a new one, we look forward to seeing you in our office. If this will be your first visit, we want to extend a warm welcome to you and help you prepare before you arrive, so your visit will go as smoothly as possible.
Preparing for Your Appointment
We understand that your time is valuable, and filling out patient forms takes extra time. That is why we have provided those forms on our website for you to download and complete before your appointment. By printing and filling them out before you arrive in our office, you will save time during check-in. Simply click the links below to open the forms. After they open, select the “print” button. Once you have the hard copies, fill them out completely and bring them with you to your appointment. The key forms we need include:

You will need the Adobe Acrobat Reader to view these files. If you don’t already have this program, you can download it for free.
When you arrive for your appointment, please bring all of the completed forms with you. We will also need your insurance cards and a photo ID for check-in. We would like a list of your current medications and the names of your current doctors as well. If you have any recent medical records or x-rays related to your feet that may be helpful, bring those along—especially if you are seeking a second opinion on a condition. If you are transferring your care from another doctor to our office, we do ask that you bring your record with you to help us understand your medical history and current care.
Please arrive fifteen minutes before your appointment to allow extra time for the check-in process, especially if you chose not fill out the patient forms before your first visit with us, or if any forms need to be updated for the year. We do our very best to respect your time and stay on schedule. As a part of that, we do not double-book our patients. New patient appointments are longer than the average check-up; typically they require 60 minutes for that first evaluation, though the length also varies with your condition. If you will need to reschedule or cancel your appointment, we ask that you let us know at least 24 hours before your appointment time.
